Case Report: Post herpes zoster comedones: A new entity under Wolf’s isotopic response

The emergence of a completely new skin condition at the precise location different, unrelated and previously healed disease is referred to as "Wolf's isotopic response". It also post-herpetic response because it typically manifests after contracting varicella-zoster virus causing herpes zoster. Wolf’s rare, unique widely accepted phenomenon. In this study, we discuss various theories proposed behind its etiology. Further studies can help us understand pathomechanism localization diseases thereby, helping manage effectively. Here, describe case 36-year-old male who presented in outpatient department with multiple open closed comedones distributed along site T2 dermatome infected by zoster infection. Dermoscopic examination revealed multiple, round, brown-colored follicular keratinous plugs peri-lesional erythema. Histopathological analysis showed large dilated canal containing orthokeratotic stratum corneum consistent comedone formation. The patient was diagnosed zosteriform prescribed oral isotretinoin 20 mg, topical tretinoin 0.05% cream moisturizer asked follow up regularly. lesions gradually over five-month period. our case, timely medical intervention helped preventing further progression disease. However, involving sample sizes identify underlying mechanism phenomenon.
